Patient information sheets

 
There are two sheets that can be downloaded. The sheets explain what a medication review clinic is, how to make an appointment and how to prepare (what to bring, questions to think about). The patient information sheets can be attached to the invitation letter or left in surgeries and pharmacies for patients to pick up. One letter is for a generic review by any health care professional
 
The second information sheet is for a pharmacist conducted review.  The issues covered are ones that have been highlighted as pre-clinic expectations/concerns by patients we have talked to in focus groups. We have found that some patients may have concerns about seeing a pharmacist for a review rather than their normal GP. Having attended a review with a pharmacist the vast majority have been happy with the outcome. However the information sheet is design to address any patients concerns.
